(HCPC), and resource support from Acenda Integrated Health, Inc. (Acenda), Food Research & Action Center (FRAC), and the Robert Wood Johnson Foundation (RWJF), is proud to showcase the transformative impact of partnership-driven system-change that is empowering some of Paterson, NJ’s most vulnerable residents. By leveraging shared expertise and resources, these organizations are—collectively—breaking down systemic silos, reducing toxic stress among high-need community members, and creating pathways for seamless, impactful service delivery at the ground level.
