The New Jersey Department of Health, Office of Minority and Multicultural Health (OMMH) is pleased to announce the Request for Applications (RFA) for the Specific Populations Community Grant for organizations serving the Black and African American population. This funding opportunity supports community-based efforts to address health disparities and improve health outcomes across New Jersey.

Letter of Interest (LOI) deadline: May 18, 2026
Organizations selected through the LOI process will be invited to submit a full application beginning May 26, 2026
All applicants must review the RFA carefully for eligibility requirements, submission instructions, timelines, and program expectations
This is a grant opportunity open to community-based and faith-based organizations with 501(c)(3) IRS status.
Funded organizations will support activities such as:
Conducting community needs assessments
Providing culturally and linguistically appropriate health education
Delivering navigation and referral services to reduce barriers to care
Collecting and reporting program data and outcomes
