The CHW Hub, in collaboration with the Hypertrophic Cardiomyopathy Association, invites you to join us for a webinar on April 9, 2026, at 12 p.m. to 1 p.m. to learn about Hypertrophic Cardiomyopathy (HCM). HCM is often "hiding in plain sight", but the clues can be in a family's heart health history. 


As a Community Health Worker, you are the first line of defense. The presentation will cover: 

  • Family Heart Health History: How to ask the right questions about "sudden deaths" or "heart failure" in a resident's family history

  • Red Flags: Identify symptoms like unexplained shortness of breath, fainting during exercise, or palpitations that may be a sign of HCM

  • Navigating the System: Tips for directing families to diagnosis, treatment and answers

  • Referrals: Directing your community to care
