New Jersey Community Health Worker Referral Gaps Survey

The CHW Hub's Curriculum, Education and Certification Committee is conducting a brief survey to better understand gaps in referral resources across the state and identify where CHW clients may be experiencing barriers to accessing services. Insights from this survey will help inform strategies to address service gaps and support the development of training opportunities that respond to the health and social needs of communities.
